Neighbourhood Health: Bringing NHS Care Closer to Home

As part of the Fit for the Future: 10 Year Health Plan for England, the newly announced NHS Neighbourhood Health is designed to deliver care closer to people’s homes, especially in the country’s most disadvantaged communities. Backed by £10 million in funding, the first wave of the programme covers 43 areas across England and aims to address long-standing health inequalities.

Neighbourhood Health is an NHS-led scheme which seeks to shift parts of care traditionally provided in hospitals into the community. Its core components include:

  • Creation of neighbourhood health teams built around general practice. These teams will bring together community nurses, hospital doctors, social care workers, pharmacists, dentists, optometrists, paramedics, social prescribers, local government and voluntary sector organisations.

  • Focus on providing end-to-end care that looks beyond single conditions to broader factors that affect health outcomes 

  • Initially targeting people with long-term conditions (such as diabetes, arthritis, angina, high blood pressure, multiple sclerosis, or epilepsy) in the areas with highest deprivation. Over time, this will expand to other priority groups.

Over the next year, the government intends to scale up neighbourhood health services more widely, learning from the first wave of 43 pilot sites which started on 9th September.
