This course will explore the Single Assessment Framework – quality statement for the SAFE key question – Safe Systems, Pathways and Transitions. We will look at the key elements of a safe system and how care services can clearly evidence compliance. We will also look a the main areas of non-compliance which are identified in CQC inspection reports.
The failings of the “care pathway” were highlighted in the CQC thematic review in 2014 entitled “Cracks in the pathway”, this found that “assessments were not comprehensive in identifying all of a person’s care needs.” and that “that the variation in how care is assessed, planned, delivered and monitored by hospitals and care homes puts people at risk of experiencing poor care.”, Yet still 10 years later these cracks still exist. There are many examples of care services failing to robustly assess peoples needs, failing to use the assessing tools correctly, poor quality care plans, not involving people in the process and many more. These not only result in ratings of requires improvement and inadequate but more importantly the result in social care staff not having the correct information to meet peoples needs and preferences. This is a breach of Regulation 9 – Person centred care, Regulation 12- Safe care and treatment and Regulation 17 – Governance.
